Pulsed field ablation (PFA) is an emerging ablation technology aimed at the adult atrial fibrillation population. Tissue ablation results from delivering pulsed electrical applications that induce an electric field that alters the cellular transmembrane potential, culminating in disruption of cell membrane integrity and eventual cell death.

The proposed safety benefits of PFA over radiofrequency ablation are centered arounds its tissue specificity for the cardiac myocyte cell membrane, thus sparing collateral damage to nearby non-cardiac structures such as the esophagus, phrenic nerve, pulmonary veins, and coronary arteries. Potential efficacy benefits of PFA over radiofrequency ablation include less dependence of catheter stability and contact force.

Several companies are actively conducting their human investigational device exemption trials to collect safety and efficacy data for pulmonary vein isolation. Most vendors are investigating “single-shot” ablation catheters that are designed for rapid circumferential pulmonary vein isolation, though focal PFA catheters are in development.
